Introduction to Lower Eyelid Lift
Before and After Blepharoplasty (Eyelid Lift Surgery)
What Is a Lower Eyelid Lift?
A lower eyelid lift is an individualized procedure that is planned based on each patient’s unique anatomy. It can include tightening and/or removal of loose skin, repositioning and/or removal of excess fat, and tightening and/or removal of sagging tissue.

The Procedure
Dr. Ho typically performs lower eyelid surgery under sedation or general anesthesia. Once it has taken effect, Dr. Ho creates a small incision. The transcutaneous incision is made right below the lash line and is used to address excess skin and fat. Another option, the transconjunctival incision, is made inside the lid and is specifically used to only address excess fat.
Once the chosen incision is made, excess tissues including protruding fat are removed as needed to tighten the undereye area. If transcutaneous incisions were made, Dr. Ho then closes the incisions with fine sutures. If transconjunctival incisions were used, those incisions do not require sutures and will close on their own.
Dr. Ho performs lower eyelid lift procedures on an outpatient basis. They can last for thirty minutes to an hour, but additional complementary procedures may prolong this.
Dr. Ho’s trifecta customized approach to lower eyelid surgery most often involves the following steps:
- Removal of fat through the transconjunctival approach: A scarless incision is created on the inside of each eyelid. Through each incision, the excess protruding fat is removed from each of the three fat compartments of the lower eyelids. This step reduces the puffiness or undereye bag appearance and leaves no outer scar.
- Facial fat transfer: Fat is harvested from the abdomen or thigh via liposuction. The fat is then refined and injected into the lower eyelids and cheeks to restore volume in these areas. This step addresses the facial volume loss and tear trough deformity that are commonly associated with aging lower eyelids. For more information on facial fat transfer, click here.
- Laser resurfacing: CO2 laser resurfacing is performed along the lower eyelids, as well as on the rest of the face, to tighten the skin. This step allows Dr. Ho to avoid creating external scars and removing skin that may distort the lower eyelid shape. For more information on CO2 laser resurfacing, click here.
Recovery After a Lower Eyelid Lift
Many patients experience bruising and swelling that will take a few weeks to resolve. You are discouraged from participating in intense exercise and other rigorous activities for two weeks to avoid complications.
Lower Eyelid Lift Cost
The cost of a lower eyelid lift will be determined based on factors like the complexity of the surgery, which will vary from patient to patient. The patient’s overall aesthetic goals for the surgery can also affect the price, as can the techniques used and the fee for anesthesia or other sedation agents. The Average range for lower eyelid surgery is $10,000-$14,000 including facility and anesthesia fees

FAQ’s
How long does a lower blepharoplasty last?
A lower eyelid lift in Philadelphia is often considered a permanent procedure as it can last a lifetime when cared for properly. The procedure removes excess fat from under the eyes that can not regenerate and thus will never return. However, results may vary depending on several factors such as age, skin elasticity, lifestyle choices, and genetics. In some patients, slight changes may become noticeable after a decade, requiring a touch-up procedure.
What is the best age for lower blepharoplasty?
While there is no perfect age to undergo under-eye surgery in Philadelphia, the majority of patients are over the age of 40. The eyes often show the earliest signs of aging for many individuals. However, men and women of any age, who are at least 18 and concerned about the appearance of their lower eyelids, may be good candidates for this procedure.
Will insurance pay for blepharoplasty?
Lower eyelid procedures are generally not covered by insurance as they are often considered cosmetic surgeries. However, in some instances, an upper blepharoplasty may be covered by insurance if the person’s vision is obstructed due to drooping eyelids. Although insurance does not cover lower blepharoplasty, many financing options are available to help make the surgery more affordable.
How painful is a lower blepharoplasty?
Most patients report very little pain after an eyelid surgery in Philadelphia. If discomfort is present, it typically peaks around 24-48 hours after surgery and is manageable with the use of over-the-counter medications. Swelling and bruising are common after this procedure and can be treated by elevating the head while sleeping and ice packs. These measures will also help to reduce any pain that may be present.
Are there risks or side effects with lower blepharoplasty?
Risks are associated with all surgical procedures and must be considered prior to undergoing a procedure. The adverse effects that can occur after a lower blepharoplasty procedure include:
- Asymmetry and scarring
- Infections and bleeding
- Dry, irritated eyes and blurred vision
- Eyelid retraction
- Vision loss
- Double vision
- Anesthetic complications
Can lower blepharoplasty be combined with other procedures?
Eyelid lift specialists in Philadelphia will use their knowledge and experience to identify areas of the face that would benefit from a cosmetic procedure. A lower blepharoplasty can be performed as a stand-alone procedure or combined with other surgical or non-surgical procedures to enhance the rejuvenation potential and limit downtime.
Can lower blepharoplasty correct under eye bags and dark circles?
Yes. A lower blepharoplasty can be effective at removing the excess fat that contributes to bags under the eyes. This reduces the appearance of dark circles giving the eyes a younger, more rejuvenated appearance. These results can be immediately noticed following the surgery, but swelling and bruising can obscure the final results, which may take months to become fully apparent.
